GUIDRY ANTITRUST SETTLEMENT FORM

Under the class action settlement in the Guidry v. Arena Football League antitrust suit, AFL players who were on an AFL team's (a) 24 man active roster, or (b) injured reserve list, for either (x) the first regular season game of an eligible season, or (y) the last regular season game of an eligible season, for at least two AFL seasons between 1996 and 2000, may be eligible to receive a settlement payment.

The AFL has supplied Class Counsel in the Guidry case with the current addresses the AFL has for players who may be eligible for such a settlement payment, and a player claim identification form has been mailed to those addresses so that Class Counsel can locate and communicate with those players. However, the AFL did not have addresses for all qualifying players and Class Counsel is not certain that all of the addresses that the AFL provided are up to date.

If you are a current or former AFL player who meets the above qualifications, and have not already filled out and returned a player claim identification form, print out the form, fill out all of the information on it, and immediately return the original, completed form by first class mail to the address indicated at the bottom of the form. Please clearly PRINT all of the information, except the required signature. Only players qualifying for a payment may fill out and submit this form, and a player will not receive his settlement payments unless he has submitted a form with the identifying information.

If you are a qualifying player, please also be sure to immediately advise Class Counsel, at the address indicated at the bottom of the form, if your mailing address or other contact information changes, always including your social security number and date-of-birth, so that Class Counsel can be assured that the information is originating from you.
